Complete your pre-assessment questionnaires

• 20 minutes somewhere quiet
• If you have old school reports or past assessments that's helpful

What you'll need

A few short questionnaires about your symptoms, your history and how things show up day to day. They give your clinician a head start before you meet.

Select two people to fill out questionnaires

• Their questionnaires completed before your assessment
• 15–20 minutes of their time to fill it out
• Their email address so we can send them the link

What you'll need

You'll select two people you know, one who knew you before age 12, and one who knows you well as an adult. Once you share their details in your patient portal, they'll each get a link to fill out a questionnaire.

Attend the assessment by secure video

  • A private space, a device with a camera and microphone, and a stable internet connection 

  • 1 – 2 hours

Meet your clinician one on one over secure video. They'll go through your questionnaires, your history and what you're experiencing.

We get it, life happens. You can reschedule or cancel free of charge up to 48 hours before your appointment. If something comes up with 24 to 48 hours' notice, a £75 fee applies, and with less than 24 hours' notice or a no-show it's £150. Either way, the rest of what you paid stays as credit toward your new booking, so you won't lose it.
